Body Mass Index (BMI) is the world’s most famous health number — quoted by doctors, gyms and insurance forms. But it’s also one of the most misunderstood. This guide explains what BMI actually measures, how the formula works, and when you should (and shouldn’t) trust it.
What is BMI?
BMI is a screening tool: a simple ratio of weight to height used to flag whether someone’s weight is in a healthy range for their height.
BMI = weight (kg) ÷ height² (m)
For imperial units, the formula becomes:
BMI = 703 × weight (lb) ÷ height² (in)
It was invented in the 1830s by the Belgian mathematician Adolphe Quetelet — not as a health measure, but as a way to describe the “average man” using statistics. It only became a public-health standard in the late 20th century, when its simplicity made it perfect for population surveys.

Where BMI falls short
BMI is a proxy for body fat, and proxies leak:
- Muscle mass. Muscle is denser than fat, so athletes and bodybuilders frequently score “overweight” or “obese” with very low body fat.
- Age. People lose muscle and gain fat with age; the same BMI can mean different things at 30 and 70.
- Ethnicity. Several studies suggest optimal BMI ranges differ for people of Asian, South Asian and Pacific Islander descent.
- Distribution. BMI can’t tell where fat sits. Fat around the organs (visceral fat) is far riskier than subcutaneous fat, and BMI is blind to it.
In short: BMI is a screening tool, not a diagnosis. It tells you who should look closer — not who has a problem.

- Waist circumference — over 102 cm (40 in) for men or 88 cm (35 in) for women signals elevated risk regardless of BMI.
- Waist-to-height ratio — keep your waist under half your height.
- Body fat percentage — measured by calipers, bioimpedance or DEXA; gives the real picture behind the BMI number.
